I am happy to bill your insurance when accurately provided to me. I do not verify your insurance benefits and/or plan information including expected copays, co-insurance and/or deductible. It would be in your best interest to do this prior to committing to therapy to avoid unexpected charges.

I am currently contracted with the following insurers: 

• Regence
• Premera
• First Choice Health Administrators
• HMA
• Kaiser
• AETNA
• Cigna
• Optum
• United Healthcare

I am NOT a State Medicaid provider and am unable to accept any of those plans.

Those utilizing insurance will be charged my contracted rate with that insurance and will be responsible for the plans corresponding deductible/copay/co-insurance rate as well as any services not covered by insurance. Copays/Co-insurances are due at time of service.

If I am not an in-network provider with your insurance, you may have Out-of-Network benefits you may want to verify. I also am a provider for several Employee Assistance Programs that your employer may be connected to for your benefit.

Fees:

Intake Assessment: $200

Individual/Family Session: $175

Sessions are typically 50-55 minutes in length.
